Chapter Event Release Form For Adults

This form is required for any non-Tacoma HOG Members 18 years or older that plan to attend a Tacoma HOG event.

Chapter Event Release Form For Minors

This form is required for all participants under the age of 18 and must be signed by the Minor Participants parents or Legal Guardian prior to the event.

Minor's Assumption of Risk Acknowledgment Form

This form is required in addition to the "Chapter Event Release Form For Minors" for any Minor attending a HOG event even if they are Tacoma HOG members.

Safe Rider Skills Reimbursement Form

This form is used to receive a $50.00 rebate from National HOG for successful completion of an accredited Riders Safety Class such as the ERC. See the form itself for more details.
